Privacy Notice

This office complies with HIPAA guidelines that ensure all medical records, medical billing and patient accounts meet strict standards with regard to documentation, handling and privacy. In addition it requires that all patients be able to access their own medical records, to correct errors or omissions, and to be informed of how their personal information is shared or used. A Notice of Privacy Practices document will be provided to all patients for their signature of acceptance for these rules of standard.
